Whenever I print an email in Outlook 2003, it prints at the top:
My Name
From
Sent
To
Cc
Subject
etc.
How do omit all that junk and just output the body of the message?

This is also not the smoothest answer, but you can open up the email, and click on the Options... button (or if it's not added to your toolbar, it's menu item View->Options...). You then can see the full internet headers, including all the full email addresses. Copy and paste whatever portion of that text you want into your reply.

I have a single customer with the same behaviors, The mailbox is an Exchange 2010 mailbox and the client software is Outlook 2010. We found that the user has sync'ed the mailbox to android tablet and iPhone. When the customer used either the tablet, iPhone or OWA to send emails, there was no duplication. We turned off both devices and the duplicate messages stopped sending. After about 3 weeks the problem reappeared. Turning off the devices for a short time resolved the issue again. There are no rules in the mailbox, hidden or otherwise. Creating a new profile does not fix.

I believe print styles can be used to change how your emails are printed

If that doesn't work for you, follow the instructions here. It gives you a few methods for doing so. The "Remove headers from the message form" is probably the most integrated method, but will change how you see emails when you jsut view them.
