Say if I clicked open 10 image files, and 2 PDF files, and quit Preview, then next time when I click on 1 image file or just start Preview, all those 12 other windows will open too… I was playing a clicking game to close them one by one… is there a quick way to make Preview not do that or "Close All"? (while keeping other apps to open the previous state).

Uncheck the Restore windows when quitting and re-opening apps.
You can also toggle this behavior by holding the option key when quitting an application.

In Mountain Lion, this option has been changed to read, "Close windows when quitting an application" with fine print to explain that open documents will not be restored.
You can duplicate pages in preview by opening up the sidebar to display thumbnails (View->Thumbnails), click on the page you want to duplicate, and then hold down Option and drag the thumbnail to the new location. That will create a duplicate of the page and leave it at the new location. When you do it this way, it does NOT duplicate the internal images and assets, but will reference them on the new page. I just confirmed with a one-page PDF which had a 400k scanned image: I turned it into a 6-page document which was still around 400k.
If you instead add the page you wish to duplicate with Edit->Insert->Page From File... it loads all the assets again and does not reference the ones you'd already loaded, causing the file size to balloon. It would certainly be nice if it could recognize that the assets are the same, but alas.

Something like RestoreMeNot or Tinker Tool can be used to selectively control which apps restore their windows and should be able to do what you want.
Alternatively, using ⌥ + ⌘ + W in Preview (and most other apps) will close all currently open windows.
