MacOS – Force Quit Keyboard Shortcut Not Working

I have a problem where the shortcut command+option+esc does not open the force quit menu. I've tried resetting the NVRAM and the SMC, but this did not fix the issue.

Also, sometimes when I reboot the computer the keyboard shortcut does open the force quit menu, but then the touchbar acts strangely: when I tap the buttons on the touch bar, I'm brought to the corresponding section in System Preferences instead (e.g. when I touch to lower brightness, I'm brought instead to the brightness section in system preferences).

So either the touch bar doesn't work properly or the force quit keyboard command does not. How do I fix this really strange behavior?

Have you tried opening the Force Quit menu from the top menu? Just click on the Apple in the top left corner and manually select Force Quit. It should open up the menu without having to use the touchbar.

Go to System Preferences -> Keyboard -> Keyboard Shortcuts -> and assign and enable the shortcuts you want.

open -a and launch seem to keep it open as well. But if you show the keyboard viewer from the input menu and close the window, the process is quit immediately. So one workaround might be to emulate clicking the menu item:

tell application "System Events" to tell process "SystemUIServer"
    tell (menu bar item 1 where description is "text input") of menu bar 1
        click
        click menu item "Show Keyboard Viewer" of menu 1
    end tell
end tell

Also, I can re-open the application after closing it via a keyboard shortcut ONLY WHEN I open the viewer via the menu button or settings.

If all windows have been closed, activate doesn't usually open a new default window. reopen would do that, but it doesn't seem to work with KeyboardViewer.
