after installing rEFInd boot manager on my macbook pro '15 w/ macOS sierra, on boot my computer takes ~20-30 seconds on a black screen then boots directly to macOS utilities. I don't know why this is happening, I am unable to boot into my regular OS. The only way I can do this is by pressing x and directly booting from the recovery screen
Second edit: It seems that when I hold space and spam enter, it will boot me into macos. Perhaps the ui is broken for refind?
Turns out resetting PRAM and NVRAM via pressing ⌘+⌥+P+R at boot (before the gray screen appears; hold until computer restarts and the startup chime has been heard twice) fixes the problem.
I have since successfully booted both OSX and Ubuntu via rEFInd without any excessive waiting.
Since you can execute a live version of Ubuntu, you have access to the gdisk command. Once booted to Ubuntu, you will need to open a Terminal window. From the Ubuntu desktop, press the control+option+T keys. A Terminal window should appear, as shown below.
You can confirm partition /dev/sda2 is a possible "Mac OS Extended" (Journaled)" volume by entering the command given below.
sudo dd if=/dev/disk1s2 count=3 | hexdump -Cv
The output at offset 00000400 should appear similar to what is shown below. What is important find is the characters HFSJ.

The interactive command gdisk can be used to change the type id of a partition. The example below assumes the partition where macOS resides is a single Mac OS Extended (Journaled) volume. The example will change type id for partition 2 to 48465300-0000-11AA-AA11-00306543ECAC. The command to enter is shown below.
sudo gdisk /dev/sda
The interactive input/output is shown below.
