I gave my iPhone 4 to my daughter and she keeps charging to my Apple account. She signs in with her own Apple ID, but I keep getting charged for apps she is buying.

What part of the iPad do you want to add a new Apple ID to? There are several different places to enter an Apple ID.
Messages
Settings > Messages > Receive at:
App Store
Settings > Store > Apple ID:
Alternatively, you can go to the App Store, click on "Top Charts" at the bottom. Scroll to the bottom and you can sign out and enter a new Apple ID.
iCloud
Go to Settings > iCloud > Account:
It's obvious Apple has a build in procedure to block this kind of actions. Otherwise they would go bankrupt... (Haha :))
However there are some work-arounds:
- eHow.com has dedicated a tutorial concerning this issue. I haven't tested it, but the intro states it will do what you've asked
- another way to do so is to just use both account on your iTunes, iPhone, iPad,... I do it with my iPad as well (both US and Belgium account) and yes it's a hassle, but it does the trick. I can access the apps, music and more from both accounts.

Go to app store, then at the bottom of the Featured page you will see the Apple ID which is used in it. She might still be using yours.
Signing with her Apple ID to iCloud is not related to the store. Your account might still be there.
