I need to be signed into iCloud and have Find my iPhone turned on. I have done both of those things and Find my iPhone locates it as well, but the device doesn't show up on my iCloud or AppleID account even though it backs up on iCloud.
What can I do to fix this?
This is almost certainly a keychain issue. Try these steps:
Hopefully this gets rid of the Approve your device notifications! Let me know how you go.
[EDIT] ***Make sure you have a passcode setup on your iCloud Keychain on your computer. If you don't set one up and then try approving your phone again. That is what made mine work. Hope this helps!
Log into www.appleid.apple.com and from here you will see all the devices currently logged into your Apple ID, you can also delete devices that you don't recognise.

I was having the same problem and in iCloud.com now it appears to be requiring iOS 8 or higher.
