My volume is turned up, it's not on mute, alarms and other things that make noise work, it's not set to "do not disturb" or anything like that. If I happen to have the iPad open when a Facetime call arrives I can see it, but it's silent and not ringing. Facetime works fine except if somebody calls me it won't ring.

Yes - this technology seems to be driven by your contact card info stored by calling party. If calling party has info for you and someone else in same contact card, then both phones will ring for FaceTime even if not same Apple ID.
In our case, we had friend who had stored my number and husband's number (not email) under my contact card. Both our phones would ring when friend tried to FaceTime me, even though husband and I have separate Apple IDs and caller never selected the other number.
Solved it by having calling friend remove the additional phone number from my contact card on his machine.
Ok, figured it out myself. My first problem was that for some reason my phone hadn't activated properly with iMessage / FaceTime (my phone number was unchecked in the You can be reached by FaceTime At section in FaceTime settings). That required switching off FaceTime and iMessage and switching them back on again after a short wait so that they could activate properly, which they did.
Once that was done, my FaceTime settings (on my iPhone) had a new option: iPhone Mobile Calls:
My wife's phone also has that setting. Choosing which phone is used is just a question of only having this setting enabled on one phone at a time. I assume (all though I haven't tried it) that if both phones are enabled in this way, FaceTime on iPad or Mac gives you the choice of which phone/number to use. It wasn't giving me a choice earlier because there was only one phone enabled.

Go to ‘Settings’, then to ‘Do Not Disturb’ and make sure it is set to allow all calls from FaceTime.
Go to ‘Settings’, then ‘Sounds’, and make sure you have a ringtone for calls / FaceTime set.
Finally, make sure your iPad is not in Mute mode by swiping from bottom of the screen upwards and tapping the ‘Bell’ icon to disable it (or, if you have an older device, and you have ‘Mute’ set under ‘General / Use side switch to...’, switch the side switch on). Having Mute mode enabled will prevent the device making any sound when called.
