I installed an app SysSecInfo and it shows 'anomalies' and 'custom jailbreak'. It doesn't explain what is anomalis about it or what it means. How do I know if my iPhone is hacked or if the app is broken?

The app hasn't been updated for iOS 10.2. It expects a previous system, so changes that have occurred as part of Apple's iOS updates are detected as anomalies. Everything looks normal in your screenshots. Furthermore, the AccessibilityBundles was always a known bug as that was shown even when they were expected.
The only irreversible thing you could do is to upgrade your baseband, eventually making it harder/impossible to unlock. In the past bricking the device was possible, because you were able to reflash the bootloader incorrectly (iPhone 2g) and thus bricking the device.
PwnageTool and Redsn0w are from the same developers team (dev-team) and they are using the same jailbreaking method (as of redsn0w 0.9.6rc9 and PwnageTool 4.3). IMHO both are safe, but PwnageTool gives you more power.
There is no Apple supported way to update an iOS devices OS except to the latest current OS release that runs on that device (or a beta OS, if it is strictly a test device and you are a registered developer). Apple no longer signs (which includes vouching for the security of) any earlier OS releases (except for old devices, such as an iPhone 3GS).
